How do I do it?

Choose two words, each at least 5 letters long
Write them down in the box below, separated by “.”. Be careful about lower- and upper-case letters, the generator is case sensitive and will generate different ID if you enter “word1” and “Word1”
Copy character sequence generated by the system and paste it in a proper field called user ID in each questionnaire.
Remember to use the same user ID anytime you are asked for it.
If you lose your user ID, just go back to generator and enter the same words you did before. The same user ID will appear.

Remember - you can fill this form as many times as you want, and each of the reported events will be recorded and assessed separately.
